Please note that you may fill out the following form electronically or on paper. After you complete the complaint form, please submit it via email at DESECompliance@mass.gov or by mail to Problem Resolution System Office, Massachusetts Department of Elementary and Secondary Education, 135 Santilli Highway, Everett, MA 02149. If submitting via email, for the signature line, you may either type your name or insert your electronic signature, both of which will be considered the legal equivalent of your manual/handwritten signature.
