LEA Assignment Request Form, Revised August 31, 2019

DESE has revised the Request for Clarification of Assignment of School District Responsibility Form (also known the LEA Assignment Request Form) to be submitted by agencies, districts, and individual seeking determination of assignment of financial and programmatic responsibility for a student's special education program where responsibilities are not clear.

The revision accomplished three goals:

  1. Facilitated some minor format restructuring.
  2. Added a block for reporting the student's Current District and Date of Enrollment.
  3. Incorporated notes clarifying ESSA requirements regarding Best Interest Determinations (BID) for students in foster care, and that a BID must occur before filing a Request for Clarification of Assignment of School District Responsibility.
